The Reduced Fare monthly sticker is valid only on a UTA issued Reduced Fare Card. You may qualify for a reduced fare card if you have a Medicare Card, receive Social Security Disability, or according to the VA are at least 40% disabled. Any other verification of a transportation disability must be determined by a licensed professional, who in turn must sign a Reduced Fare Card application. You may obtain an application by going to one of our local UTA Customer Service Offices located at 3600 S. 700 W., West Valley City; 24 W. 100 S., Salt Lake City; 1145 S. 750 E., Orem; or 2393 Wall Ave., Ogden. The sticker is not valid on any other card and will be confiscated if improperly used. NO REFUNDS WILL BE ISSUED FOR THOSE NOT QUALIFYING FOR THIS MONTHLY STICKER. |
